Capital Healthcare Associates, a division of Community Care Physicians, has provided medical services to the Capital Region for over thirty years. Our physicians maintain certifications in their respective specialties of internal medicine, geriatric medicine, family medicine, occupational and preventive medicine. Our adjunct providers include nurse practitioners and physicians assistants who are likewise registered and certified in their field. Our entire staff is dedicated to providing our patients with quality, convenient and compassionate health care.

What We Do
Primary Care. It’s your first stop for getting healthy and staying healthy. We provide all aspects of primary care for adults. We provide office based care, and facility based care at a number of long term geriatric care facilities where we also hold medical directorships.
