Please call our office at (518) 274-9126 during the week to schedule an appointment. After hours emergency calls will be handled by the on-call physician.
Monday: 7:30 am – 4:30 pm
Tuesday: 7:30 am – 4:30 pm
Wednesday: 7:30 am – 4:30 pm
Thursday: 7:30 am – 4:30 pm
Friday: 7:30 am – 4:30 pm

Primary Care Physician
You select your primary care physician. Your primary care physician is in charge of a health care team that provides your regularly scheduled mecical care. Your primary care physician will also provide care for illnesses or injuries that arise between scheduled visits. This team includes physician assistants, nurse practitioners, nurses, and the office support staff.
All of our health care providers — physicians, physician assistants, and nurse practitioners, are trained and certified in the diagnosis and treatment of medical problems. Together with our office support staff, they are focused on providing you the care you need when you need it.
Your complete medical record is available to each provider within Community Care Physicians, so all are informed with the information they need to care for you.
Cancelling an Appointment
If you must cancel an appointment, we ask for 24 hours of advance notice. We are happy to reschedule your appointment.
We contact every patient multiple times with appointment reminders before your appointment.
We do charge a fee for patients who miss their appointment and who have not called to cancel.
